Visitor Health and Safety Guidelines

Masks are recommended for all staff and visitors. Staff members and visitors who are not fully vaccinated must maintain physical distance and present a negative test result from within 72 hours to access the campus.

The health and safety of our staff and visitors is important to us.

Visitors who are required to check in for a badge must attest to being fully vaccinated or show a negative COVID-19 test result (from within 72 hours), and complete a daily health check, to enter the main campus and other APL facilities. Only a professionally administered rapid test or polymerase chain reaction (PCR) test will be accepted; at-home or self-administered tests are not acceptable. Visitors should complete this survey within a week of their expected visit.

Visitors with a take-home picture badge (not required to check in on arrival) must be vaccinated to enter the main campus and other APL facilities. These visitors can use this FedRAMP-certified secure form to enter proof of their vaccination status.

All persons visiting the Laboratory need to be free from any illness or COVID-like symptoms and currently must not be quarantined for any reason.

To avoid delays and streamline the entry process, all visitors should check with their APL hosts about advance registration.

Hosts of any event held at APL facilities outside of the security perimeter must ensure compliance with vaccination, testing, masking, or social-distancing requirements in effect at the time of the event. While the Lab expects outside (non-APL) hosts of events at the Kossiakoff Center and other facilities to abide by APL requirements, the method they use is at their discretion. Event organizers can use the APL survey application — or scan the QR codes posted at all entrances — or use their own means to verify vaccination status or COVID-19 test results.

Mandatory COVID-19 Questionnaire for All Visitors

Question 1: Have you tested positive for COVID-19 in the past 10 days, but have not yet been cleared to return to work?

Question 2: Have you had a known exposure to a COVID positive person in the last 5 days? Have you been directed to quarantine for any reason, including a suspected COVID-19 infection, exposure to a COVID-19 case, or because of travel?

Question 3: Have you experienced ANY of the following symptoms of COVID-19 infection in the last five days?

  • Fever or chills
  • Cough
  • Shortness of breath/difficulty breathing
  • Fatigue
  • Muscle or body aches
  • New loss of taste or smell
  • Sore throat
  • Congestion or runny nose
  • Nausea, vomiting, or diarrhea
  • Unusual headache

If your answer is “yes” to any of these questions, per APL policy and current state and health agency guidance, you will not be allowed access to the facility.
